Biomedical engineers and diabetes doctors from the University of the North Carolina (UNC) School of Medicine and North Carolina State have developed an insulin patch that is capable of detecting increases in blood sugar levels and secreting insulin doses into the bloodstream when needed. The research was announced on June 22, 2015, and was published in the Proceedings of the National Academy of Sciences.

The patch is a thin square no larger than a penny. It contains more than 100 tiny needles that are approximately the size of eyelash. The “microneedles” contain microscopic storage units for insulin. The units contain enzymes that sense glucose levels and rapidly release insulin when blood sugar levels become too high. The microneedles penetrate the skin’s surface to tap into the blood flow through the capillaries just below the skin surface. In a mouse model of type 1 diabetes, the patch lowered blood glucose for up to nine hours.

We have designed a patch for diabetes that works fast, is easy to use, and is made from nontoxic, biocompatible materials,” said co-senior study author Zhen Gu, PhD, a professor in the Joint UNC/NC State Department of Biomedical Engineering. “The whole system can be personalized to account for a diabetic’s weight and sensitivity to insulin so we could make the smart patch even smarter.”

While the approach does show great promise, researchers say that more pre-clinical tests and subsequent clinical trials are needed before the patch can be administered to human patients. “Injecting the wrong amount of medication can lead to significant complications like blindness and limb amputations, or even more disastrous consequences such as diabetic comas and death,” John Buse, MD, PhD, co-author of the study and the director of the UNC Diabetes Care Center.

Researchers gave one set of mice a standard insulin injection and measured blood glucose levels that returned to normal and then quickly rose into the hyperglycemic range. They tested another set of mice with the patch and observed that blood glucose levels were brought under control within thirty minutes and stayed level for several hours. The researchers also found that the patch could be fine-tuned to change blood glucose levels in a certain range by changing the dose of the enzyme contained in each microneedle. They found the patch was not as hazardous as insulin injections, which can cause blood glucose levels to plummet to dangerously low levels if administered too often.

The hard part of diabetes care is not the insulin shots, or the blood sugar checks, or the diet but the fact that you have to do them all several times a day every day for the rest of your life, said Buse, the director of the North Carolina Translational and Clinical Sciences (NC TraCS) Institute and past president of the American Diabetes Association. “If we can get these patches to work in people, it will be a game changer.”

1: Stay Hydrated

You can lose a lot of liquid in diarrhea, but you also lose electrolytes, minerals such as sodium and potassium that are important for your body. Over-the-counter electrolyte replacement formulas like Rehydralyte are available without a prescription, or simply sports drinks like Gatorade can be used. Some juices are good, but stay away from prune, apple and pear juices because they can make your condition worse. Make sure to consume at least 8 cups of fluids, including water, to replace fluids you lose. Avoid coffee or alcohol! 2: Take OTC Medicine

Loud music puts a billion young people at risk for deafness

We all love our iPods and MP3 players. But recently a great number of audiologists have signaled the warning that the decibel level at which young people, that is those in the age range of 12 to 35, are increasingly at risk for going deaf.

The World Health Organization recently published a study which concluded that “1.1 billion teenagers and young adults are at risk of hearing loss due to the unsafe use of personal audio devices.” Add to that the threat posed by excessively loud concerts and other venues, like sporting events, dance clubs and bars, and it creates a perfect storm of conditions to deprive young people of their hearing very early in life.

Although the study was completed in February, the news only made it to the media this week. All the major main stream media outlets ran stories about the WHO findings. Until recently deafness was a problem for children in underdeveloped who were susceptible to ear infections which if not treated properly lead to diminished hearing and even deafness.

But now the problem of deafness in young people has moved to the richest most developed nations. In 2014 the National Institutes of Health published a warning about what it called “noise induced hearing loss.” Few paid any attention to the warning until the media picked up on the findings of WHO.

It all goes back to decibel levels

The WHO study, and most of the media coverage, has zeroed in on ear buds as a leading culprit of hearing loss. Unlike older ear phones which covered the ear, the newer ear buds sit directly in the ear canal. The closer the proximity to the stereocilia, the little hairs that ultimately carry electrical signals to the brain, the more quickly the stereocilia is damaged. Once damaged, hearing loss is likely to be permanent.

According to most experts, including WHO and the NIH, 85 decibels is the safest, loud level at which sound should be transmitted. 85 decibels is approximately the sound of heavy traffic. An MP3 player at maximum volume can generate over 100 decibels which is comparable to concerts, dance clubs and sporting events.

Even everyday items should be considered dangerous. Ear protection is recommended before using a lawn mower, leaf blower or even riding a motorcycle.

Timing is also critical

While the decibel level indicates sounds which may cause damage, one must also put the amount of exposure time into the equation. A short burst of sound, say a firecracker or a gun firing, has a high decibel level, but it may be less damaging then long time exposure to sounds with slightly lower decibel levels. WHO recommends that at 85 decibels, exposure to the sound should be limited to 8 hours a day.

Perhaps the most troubling problem will be disconnecting young people from ear buds which are used continually during the day and even worn to bed at night.

Apple and other manufacturers have offered apps which allow parents to set decibel levels on phone and audio players. But even with levels set low, there should be frequent breaks from the constant sound.

The WHO estimates that throughout the world at least 350 million people have “moderate to profound hearing loss.” Almost half of these are a result of disease or genetic conditions. But 50% of the remaining hearing losses are deemed avoidable.

Recent research has shown the importance of sleep for a variety of health reasons, but new research is showing that lack of sleep or waking up several times during the night may increase your chances of developing Alzheimer’s.

Alzheimer’s disease is a type of dementia that creates issues with thinking, behavior and memory. A sticky plaque called beta-amyloid builds up in the spaces between the nerve cells, causing toxicity in the brain. A study in the journal JAMA Neurology has discovered that disrupted sleep can actually increase this plaque in the brain.

Subjects over the age of 70 were studied as part of the Baltimore Longitudinal Study of Aging. Those who reported getting the least amount of sleep (less than five hours per night) and those who reported experiencing waking multiple times during the night were discovered to have more beta-amyloid plaque buildup once imaging scans were performed of their brains. This increased buildup could be a key indicator for Alzheimer’s disease risk.

Sleep studies have also discovered that those with sleep apnea (a sleep disorder that is characterized by pausing in breathing during sleep) have double the risk of developing Alzheimer’s disease.

The good news is that troubled sleep or trouble staying asleep (also known as sleep maintenance insomnia) is treatable, and this new information could be vital in protecting seniors’ brains. The average adult should be getting seven to eight hours of sleep per night. It is well known that troubled sleep causes difficulties in focus and learning the following day, and we are coming to find out this could be related to dying brain cells.

This importance of this work lies in overwhelming amount of expected Alzheimer’s cases to develop in the coming years as the baby boomer generation will be approaching their 70’s. More than 5 million Americans have already been diagnosed with Alzheimer’s disease, and that number is expected to climb to more than double in the United States by the year 2050, and according to the World Health Organization, more than triple globally, reaching an estimated 115.4 million cases.

Nearly one in five adults over 65 drink daily, not without risk

A July 2014 study by consumer analysts, The Mintel Group, concludes 18 percent of people over age of 65 drink daily. The Centers for Disease Control and Prevention (CDC) says one in ten in this age group is a binge drinker. Not every daily or binge drinker is alcoholic, however there are considerations aside from the disease of alcoholism:

  • There’s a three times higher rate of depression compared to non-drinking seniors;
  • Drinkers have a three times greater risk of hip fracture; and
  • Drinkers past the retirement age have a 16 times greater likelihood of a suicide attempt compared to a non-drinker.

There are drug interactions as well, as highlighted in a previous episode, and as people age they generally are prescribed more medications and take more over the counter remedies. All in, more than 120 meds have an adverse reaction to alcohol.

Statistics on alcohol-related deaths in older adults are not entirely accurate, although they point to a higher toll than the under-60 crowd. Take for example, a retiree who has a wicked fall following a drinking episode or an alcohol/drug interaction. The medical examiner’s “cause of death” would be a head injury. What caused the head injury? Or more to the point, what caused the fall? Back to an earlier stat: Why do drinkers have a higher likelihood of hip fracture? Not just because old age weakens bone, but because alcohol compromises an already compromised sense of balance.

A July 2015 British Medical Journal study noted that those over age 50 who are active, social and well-off are drinking more – a “middle class phenomenon” which may be a hidden health and social problem in otherwise successful older people. What may skew all the stats on the extent of drinking in older adults is that maybe they’re drinking the same as ever before (which would be bad news or no news) but now they’re wise enough to admit to problem drinking and seek help for alcohol use disorders that nobody solves on their own (and that’s good news).
